Coordinated Family
Focused Care (CFFC)
at Worcester Communities of Care
WCC is proud to be one of five pilot sites in the
Commonwealth for the Coordinated Family-Focused Care (CFFC) initiative.
CFFC is a model of care jointly funded by Mass. Health Behavioral Health
and the Departments of Mental Health, Social Services, Youth Services,
and Education. The program is managed by the Massachusetts
Behavioral Health Partnership.
The goal of CFFC is to better coordinate the care of
children and adolescents who are at risk of hospitalization or
residential placement because of their serious emotional disturbance.
The CFFC program builds on family strengths and available support
systems to help children remain or return to the community.
CFFC CAN HELP
FAMILIES:

Apply positive approaches to their child's
behavioral issues so that the child is more successful at home and
in school.

Understand the behavioral health system and receive
more appropriate services.

Resolve crises that could result in hospitalization
by making CFFC staff available 24 hours a day, seven days a week for
crisis support.

Connect with other families through peer support
groups to share their experiences and concerns.
WHO IS ELIGIBLE?
A child may be eligible if he or she has a serious emotional disturbance
that significantly affects functioning at home, school, or in the
community. The child must also be:

Between the ages of 3 to 18 years old (inclusive).
The child can be up to age 22 if he or she is receiving special
education services and has a serious emotional disturbance.

At risk of psychiatric hospitalization or
residential care.

A MassHealth member and enrolled or eligible to be
enrolled in the Massachusetts Behavioral Health Partnership.

A resident within one of the following cities or
towns: Brockton, Lawrence, New Bedford, Springfield, or
Worcester.

Other eligibility criteria apply and will be
explained by CFFC program staff.
REFERRALS:
Referrals are accepted from any source, including
families.
HOW CFFC WORKS:
A two-person team works closely with a small group of
children and their families:
The care manager is a licensed
mental health clinician, who coordinates services and supports offered
to the child and family
The family partner is a parent of a
child with a history of mental health needs who has been hired to be
part of the professional staff and who provides support and advocacy
services to the family.
After the child is accepted into CFFC, the program staff meets with the
child and family to explain the model and to conduct a thorough
strengths-based assessment of the child's and family's needs.
The CFFC staff then works with the family to develop the child's
Care
Planning Team (CPT), which consists of the child, family,
people invited by the family, CFFC staff, and appropriate state agency
and school staff.
The Care Planning Team develops an
Individual Care Plan,
a plan that focuses on the child's
needs and proposes ways to improve the child's well-being in all
settings, including at home, at school, and in the community. The
Individual Care Plan serves as the roadmap for the child, family and
other Care Planning Team members and helps them to work together to
better support the child and family in the achievement of their goals.
