CMCC Initiative |
One of the major activities of the CMCC Grant will be the establishment of two Family Centers. Although each Family Center will have a primary location in South and North Central Massachusetts, CMCC is committed to providing supportive services within the surrounding communities.
The Family Centers will be the primary door through which families and youth come to receive support and assistance around the myriad of issues that most often lead to CHINS and/or court involvement. Given the reasons for CHINS and/or court involvement can vary from youth to youth, it is important to first get a good understanding of the youth and family needs in order to successfully help.
Supportive groups, classes, and activities will be provided initially by the Assistant Family Center Director and Youth Coordinator in each site. As families become involved in the Family Center, the Assistant Director will coordinate an active volunteer program in which parents and youth will be trained and supervised to provide groups, classes, and services to other youth and families.
Care Facilitation will be conducted by Care Partners and supervised by the Family Center Director, who will be a licensed clinical professional. Care Partners, who will be parents of youth with SED themselves, will be trained in the Wraparound Service Planning Process, and will assist families with the formation of a Care Planning Team and the creation of a Care Plan which will identify family and youth strengths, needs, and a plan for meeting those needs.
